Directions Driving Directions: Northbound on I-405:•Take Exit 20-B [NE 124th St]. Stay left on ramp to go to NE 124th.• At the light, turn left onto NE 124th St.• Cross the bridge. At the second light, turn right onto 116th Ave NE• At the crest of the hill, turn left at the light onto NE 128th St..• At the second driveway, turn left into the parking lot at 11415 NE 128th St.
Southbound on I-405:• Take Exit 20 [Totem Lake/NE 124th St] and remain in the right-hand lane. Take Exit 20 [Totem Lake/NE 124th St] and remain in the right-hand lane.• At the second driveway, turn left into the parking lot at 11415 NE 128th St.• At the top of the ramp turn left if Northbound, right if Southbound onto NE 128th St.• At the second driveway, turn left into the parking lot at 11415 NE 128th St. Park wherever you wish, and enter building from the upper parking lot.

Once inside: Evergreen Health and Wellness Offices, LLC, Suite 100, is immediately on your right as you enter the lobby. Enter the waiting room and press the #3 button found in the waiting room to indicate your arrival. I will come to meet you at your appointment time.
**To find the restrooms, go back through the lobby and down the hallway to the left of the stairs. Restrooms are just around the corner from the Fish Tanks**
